Job summary

Angels Care Home Health is seeking a Licensed Practical Nurse (LPN) to deliver skilled nursing care in clients' homes under the supervision of a registered nurse, coordinating care with the interdisciplinary team and families.

The ideal candidate is a graduate of an accredited practical nursing program, holds an active LPN license, has a valid state driver’s license, and brings at least two years of LPN/home health experience, CPR and hepatitis B verification, and a commitment to compassionate

Qualifications

• Graduate of an accredited school of practical/vocational nursing.
• Current state license as an LPN.
• Current State’s Driver’s License.
• Two years’ experience as an LPN, home health preferred.
• Proof of current CPR and Hepatitis profile.

Responsibilities

• Administer skilled nursing care under the supervision of a registered nurse in clients' homes.
• Coordinate care with the interdisciplinary team and patient/family to support safe, effective care.
