What you will do:

• Develop and implement a comprehensive plan of care, including preventative and rehabilitative nursing procedures in accordance with MD orders. Identify problems and establish measurable goals.
• Coordinate the plan of care with team members and other agencies to ensure continuity of care and effective implementation of the plan.

Who you are:

• Current, active LVN License in the State of Arizona by the Board of Registered Nursing.
• Current CPR certification, health clearance, and clinical skills self-assessment.
• Minimum of two years of professional nursing experience.
• Minimum of two years of experience as a clinical supervisor or charge nurse in a hospital or home care setting.
