Job summary

Adoration Home Health, part of Adoration Health, seeks a Licensed Practical Nurse for PRN home health in Murfreesboro, TN. You will provide patient-centered, 1:1 care in the home setting, following each plan of care and maintaining patient dignity and independence.

Responsibilities include administering medications and treatments, documenting actions, and coordinating supplies while communicating with families and the care team. A valid LPN license and driver’s license are required.

Qualifications

• Licensed Practical Nurse with unrestricted state registration.
• CPR certification required.
• Driver’s license and clean driving record.
• 1+ years nursing experience preferred.
• Home health care experience preferred.

Responsibilities

• Provide 1:1 client care per plan of care while preserving dignity and independence.
• Administer medications, feedings, oxygen, and ostomy care as directed.
• Document nursing actions and communicate changes to family, case manager, and physician.
• Coordinate or order supplies and medications as needed to ensure adequate stock.
• Ensure patient needs are met per physician instructions.

Responsibilities

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
• Accurately documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting the progress and outcomes for established goals and informing physician, case manager, and families of changes in client's medical condition and needs
• Coordinating or ordering supplies, medications, and feedings as needed to insure adequate amounts are always on hand
• Ensuring all patient needs are being met in accordance with the instructions of the physician

Qualifications

• LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice
• Valid driver's license, acceptable driving record and proof of car insurance
• CPR professional certification
• 1+ years of nursing experience preferred
• Home H ealth care experience preferred
