Adoration Health in Murfreesboro, TN is seeking a compassionate Licensed Practical Nurse to join our Home Health team on a PRN schedule. You will provide 1:1 patient‑centered care in the home, following each client’s plan of care and communicating with families and physicians.

The role requires an LPN license, CPR certification, a valid driver’s license, and 1+ year of nursing experience. We offer flexible scheduling, mileage reimbursement, and a comprehensive benefits package.

Qualifications

• LPN/LPT/LVN license in the state of practice is required.
• CPR certification is required for clinical duties.
• Valid driver’s license with reliable transportation.
• 1+ year of nursing experience; home health experience preferred.

Responsibilities

• Provide 1:1 home health nursing per the patient’s plan of care.
• Administer medications, treatments, and assessments.
• Document nursing actions and communication with families, case managers, and physicians.
• Coordinate supplies, medications, and feeds as needed.
• Maintain patient safety and dignity while delivering care.

Responsibilities

As a Home Health Licensed Practical Nurse, You will:

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
• Accurately documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting the progress and outcomes for established goals and informing physician, case manager, and families of changes in client's medical condition and needs
• Coordinating or ordering supplies, medications, and feedings as needed to insure adequate amounts are always on hand
• Ensuring all patient needs are being met in accordance with the instructions of the physician

Qualifications

• LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice
• Valid driver’s license, acceptable driving record and proof of car insurance
• CPR professional certification
• 1+ years of nursing experience preferredHome H ealth care experience preferred
