Summa Home Health is seeking an experienced Licensed Practical Nurse (LPN) for full-time, in-home patient care in Akron, OH. You will provide one-on-one skilled nursing, follow orders, monitor conditions, administer medications, and educate patients and families to support recovery at home.

You will work with an interdisciplinary team to reduce readmissions, maintain detailed clinical documentation, and ensure timely communication of changes in schedule or patient status.

Qualifications

• Current LPN license required.
• Minimum 1 year of LPN experience in an acute care setting.
• Home health experience preferred.
• Valid driver’s license and active auto insurance.
• Strong communication, time management, and documentation skills.
• Compassionate, dependable, and patient-focused.

Responsabilités

• Provide skilled nursing care in patients’ homes.
• Follow physician orders and care plans created by the Case Manager.
• Monitor patient conditions and report changes.
• Administer medications and treatments as prescribed.
• Educate patients and families using teach-back methods.
• Help reduce hospital readmissions through patient education and preventative care.
• Complete clinical documentation in accordance with agency and Medicare guidelines.
• Communicate schedule or availability changes with the appropriate team members.
