Qualifications

• Current and unrestricted LPN licensure in the state of Washington.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Excellent communication, writing, and organizational skills.

Responsibilities

• Provides high quality clinical services within the scope of practice as defined by state laws and plan of care.
• Documents patient visits thoroughly in the electronic medical record.
• Participates in care coordination including reporting needs and changes in condition.
• Function in home environments regardless of client demographics.
