Job summary

Optum in Moses Lake (Assured Home Health) is seeking a Licensed Practical Nurse to provide high-quality, in-home nursing care under the plan of care, coordinating with the health care team and family to support patient wellbeing.

This full-time role requires Washington LPN licensure and reliable transportation; a $2,500 sign-on bonus is available for eligible candidates, with a comprehensive benefits package and opportunities for professional growth.

Qualifications

• Current and unrestricted LPN licensure in the state of Washington.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred CPR certification or ability to complete within 90 days of hire.
• Home care experience is preferred.

Responsibilities

• Provides high quality clinical services within the scope of practice as defined by state laws and plan of care.
• Documents patient visits thoroughly in the electronic medical record.
• Participates in care coordination, reporting needs and changes in condition to the team.
• Function in any home situation regardless of age, race, creed, color, sex, or financial condition.
