Job summary

Medi Home Health & Hospice, a division of Medical Services of America, is seeking a full-time Licensed Practical Nurse (LPN) to serve Richlands, Tazewell, and Bluefield, VA areas. Provide in-home, skilled nursing care under RN supervision to support patient independence and reduce hospital visits.

Requires completion of an accredited practical nursing program, Virginia LPN license, and at least 1 year of clinical nursing experience (home health preferred). Competitive benefits offered.

Qualifications

• Completion of an accredited practical nursing program.
• Current LPN license in the state of Virginia.
• Minimum of 1 year of clinical nursing experience (home health preferred).

Responsibilities

• Deliver one-on-one skilled nursing care to home health patients under RN/physician plan.
• Support the development and execution of individualized care plans.
• Monitor patient progress and document clinical updates and observations.
• Collaborate with the RN and healthcare team to ensure continuity of care.
• Educate patients and caregivers on basic care techniques and routines.
• Participate in the agency’s on-call rotation for care coverage.
• Maintain compliance with agency policies and procedures.
