Job summary

Phoenix Home Health is hiring a licensed practical nurse in Springfield, MO to provide skilled nursing care in patients’ homes Monday through Friday, 8:00 AM–5:00 PM, with occasional weekend rotation. The role emphasizes one-on-one patient care with AI-assisted charting to minimize charting time.

You’ll assess, educate, and support patients and families, collaborating with an interdisciplinary team to promote recovery and independence at home.

Qualifications

• Current Missouri or Compact LPN license.
• Excellent assessment and communication skills.
• Strong organizational and time management abilities.
• Compassionate, patient-centered approach to care.
• Valid driver’s license and reliable transportation.
• Home health experience is a plus, but we welcome nurses eager to learn.

Responsibilities

• Deliver skilled nursing care in patients' homes according to physician orders and individualized care plans.
• Perform comprehensive patient assessments and monitor changes in condition.
• Educate patients and families to promote confidence and independence at home.
• Collaborate with RNs, therapists, physicians and the care team.
• Maintain accurate documentation using AI-supported charting technology.
• Participate in quality improvement initiatives and ongoing professional development.
