Phoenix Home Health in Missouri’s Lincoln County and surrounding areas seeks a licensed practical nurse for a full-time, Monday–Friday day shift. On-site charting support and collaboration with a multi-disciplinary team ensure comprehensive patient care.

Salary ranges from $50,000 to $80,000 annually with benefits and orientation. You will perform physical assessments, follow physician orders, adjust care plans as needed, and educate clients and families.

Qualifikationen

• Exceptional skills in physical assessment and communication.
• Ability to execute tailored care plans based on physician orders.
• Collaborates with nurses and therapists to provide comprehensive home health care.

Aufgaben

• Review and revise the plan of care.
• Provide nursing care per physician's plan of treatment.
• Observe, record, and report patient changes to appropriate personnel.
• Provide ongoing instruction to client or family in the plan of treatment.
• Participate in education programs and nurse meetings.
• Monday through Friday 8am to 5pm with every 4 to 6 weeks, weekend rotation.
