Job summary

Assisted Home Health & Hospice in Peoria, AZ is seeking a full-time LVN to join our expanding Home Health Division. You will develop and implement comprehensive care plans, coordinate with the care team, and help patients achieve optimal outcomes.

The ideal candidate holds an active AZ LPN license, has CPR certification, and at least two years of nursing experience, including supervisory roles. Join us to grow with a supportive team and patient-centered care.

Qualifications

• Must hold an active LPN license in Arizona.
• Current CPR certification and health clearance.
• Minimum two years of professional nursing experience; supervisory/charge nurse experience preferred.

Responsibilities

• Develops and implements a comprehensive plan of care per MD orders, including preventive and rehabilitative nursing procedures.
• Coordinates the plan of care with the care provider team and other agencies to ensure continuity of care.

What you will do:

• Develops and implements a comprehensive plan of care including preventative and rehabilitative nursing procedures where appropriate in accordance with MD orders. Identifies problems and establishes measurable goals
• Coordinates the plan of care with members of the care provider team and other agencies providing nursing or related services to provide continuity of care and to implement a comprehensive plan of care

Who you are:

• Current, active LPN License in the State of Arizonaby the Board of Registered Nursing.
• Current CPR, health clearance and clinical skills self-assessment.
• Minimum of two years of professional nursing experience.
• Minimum of two years of experience as a clinical supervisor or charge nurse in a hospital or home care setting.
