Job summary

Csmedicalpro is seeking an LPN for a contract Home Health role in Fort Wayne, IN. The LPN will provide direct and indirect patient care in home settings under the plan of care and will collaborate with the RN to deliver nursing treatments.

Responsibilities include infusion, wound care, venipuncture, phlebotomy, ostomy, and catheter care, plus teaching patients and caregivers to live independently at home. Must pass background check, drug screen, and provide updated immunizations.

Qualifications

• Must be a licensed Practical Nurse (LPN).
• Background check, drug screen, and updated immunizations required.
• HCHB/PointCare experience in Home Health preferred.

Responsibilities

Preferred experience with HCHB/Pointcare in Home Health.The Licensed Practical Nurse is a licensed professional who provides direct and indirect patient care services to patients in their home environment or alternative setting. The LPN collaborates with the Registered Nurse to provide nursing treatments to patients according to the plan of care established. The LPN provides technical nursing interventions including infusion, wound care, phlebotomy, ostomy, catheter care, and other skilled interventions as ordered by the physician and within their Scope of Practice.
