Job summary

A-Line Staffing Solutions is seeking a Licensed Practical Nurse (LPN) for home health in Fort Wayne, Indiana. The role provides direct and indirect patient care in home and alternate settings, collaborating with a Registered Nurse to implement care plans.

This onsite position offers a 40-hour work week, M-F, with mileage reimbursement and a path to benefits after 90 days and a 401(k) after one year.

Qualifications

• Basic nursing skills including venipuncture, wound care, disease management teaching.
• Licensed Practical Nurse (LPN) license required.
• Preferred experience with Home Health software (HCHB/Pointcare).

Responsibilities

• Provide direct and indirect patient care in home or alternate settings.
• Collaborate with Registered Nurse to implement nursing treatments per care plans.
• Perform technical nursing interventions such as infusion, wound care, phlebotomy, ostomy, and catheter care.
• Instruct, motivate, and assist patients and caregivers to improve independence at home.
• Maintain documentation and ensure compliance with healthcare standards and policies.

Critical Information

• Shift: M-F 0800-1700; 40 hours guaranteed
• Remote/on-site/hybrid: Onsite
• Mileage reimbursement: 0.47 cents per mile
