Adoration Health in Nashville, TN is seeking a Home Health Licensed Practical Nurse to join our team. This full-time role focuses on delivering patient-centered 1:1 home care in Davidson County, with a $2500 sign-on bonus and strong team support.

Ideal candidates hold an LPN/LPN/LVN degree, CPR certification, and a valid driver’s license, with 1+ year of nursing experience and home health exposure preferred.

Qualifications

• LPN/LPT/LVN license active in state of practice.
• CPR certification required.
• Valid driver’s license and proof of insurance.
• 1+ year nursing experience preferred, home health preferred.

Responsabilités

• Provide 1:1 home health care following the plan of care.
• Administer medications, feedings, oxygen, ostomy care as needed.
• Document nursing actions and communicate with family and care team.
• Coordinate supplies and medications to ensure adequate stock.
• Meet patient needs per physician instructions.

Responsibilities

As a Home Health Licensed Practical Nurse, You will:

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
• Accurately documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting the progress and outcomes for established goals and informing physician, case manager, and families of changes in client's medical condition and needs
• Coordinating or ordering supplies, medications, and feedings as needed to insure adequate amounts are always on hand
• Ensuring all patient needs are being met in accordance with the instructions of the physician

Qualifications

• LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice
• Valid driver's license, acceptable driving record and proof of car insurance
• CPR professional certification
• 1+ years of nursing experience preferred
• Home Health care experience preferred
