Job summary

Empath Home Health, a member of Empath Health, is seeking a full-time Home Health LPN to deliver individualized in-home nursing care in Sarasota County, Florida. The role requires 40 hours per week, Monday–Friday, with weekend and holiday rotations.

You will work under the supervision of a Registered Nurse, providing skilled nursing services, patient education, and care coordination with an interdisciplinary team to ensure holistic, value-driven care at home.

Qualifications

• Active Florida LPN license.
• Graduate of an approved nursing program.
• 1–2 years of LPN experience; home health experience preferred.
• IV certification strongly preferred.

Responsibilities

• Provides nursing services in compliance with agency policy and procedure, state and federal regulations, and the patient’s plan of care under the direction and supervision of a Registered Nurse.
• Observes patients and reports adverse reactions to medications or treatment to the Clinical Coordinator.
• Assists the Physician and/or Registered Nurse in carrying out the plan of care and in performing specialized procedures.
• Performs patient and family instruction on diet, medication, and treatment.
• Collaborate with the interdisciplinary team to ensure seamless, holistic care

What You'll Do

What You’ll Need

• Active Florida LPN license
• Applicants must be a graduate of an approved nursing program
• Requires 1-2 years of LPN experience
• Home health care experience preferred.
• Intravenous (IV) Certification strongly preferred.
