Job summary

Empath Home Health in Naples, FL is seeking a full-time Home Health LPN to deliver individualized care in the Collier County area. The role requires 40 hours per week, Monday to Friday, with weekend and holiday rotations.

You will provide nursing services under the supervision of a Registered Nurse, observe patients, report adverse reactions, assist with the plan of care, and educate patients and families on diet, medications, and treatments. Join a mission-driven team focused on Full Life Care.

Qualifications

• Graduate of an approved nursing program.
• Requires 1-2 years of LPN experience.
• Home health care experience preferred.
• IV certification strongly preferred.

Responsibilities

• Provides nursing services in accordance with policy, state/federal regulations, and patient plan of care under RN supervision.
• Observes patients and reports adverse reactions to meds or treatment to Clinical Coordinator.
• Assists Physician/Registered Nurse in carrying out the plan of care and in performing procedures.
• Performs patient and family instruction on diet, medication, and treatment.
• Collaborates with the interdisciplinary team to ensure holistic care.

What you'll Do

• Provides nursing services in compliance with agency policy and procedure, state and federal regulations, and the patient’s plan of care under the direction and supervision of a Registered Nurse.
• Observes patients and report adverse reactions to medication or treatment to the Clinical Coordinator.
• Assists the Physician and/or Registered Nurse in carrying out the plan of care and in performing specialized procedures.
• Performs patient and family instruction on diet, medication, and treatment.
• Collaborate with the interdisciplinary team to ensure seamless, holistic care
