Job summary

Freedom at Home is seeking a compassionate Licensed Practical Nurse (LPN) to provide skilled nursing care in patients' homes under the supervision of an RN. You will follow Plans of Care, perform nursing visits, and document promptly to support quality patient outcomes.

Qualifications

• Graduate of an accredited Licensed Practical Nursing program.
• Current, unrestricted Ohio LPN license or eligibility.
• Current CPR certification.
• Minimum of one year of LPN experience.
• Home Health experience is preferred but not required.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide skilled nursing care in patients' homes under the supervision of a Registered Nurse.
• Follow individualized Plans of Care established by the supervising RN.
• Perform skilled nursing visits and complete accurate, timely clinical documentation.
• Monitor patient status and communicate changes to the supervising RN and healthcare team.
• Educate patients and caregivers on medications, treatments, disease management, and self-care.
• Reinforce the Home Health Aide plan of care and collaborate with the interdisciplinary team to ensure quality outcomes.
• Participate in case conferences, staff meetings, and quality improvement activities.
• Promote safe, compassionate, high-quality patient care while helping patients remain at home.
