REPORTS TO

Registered Nurse

BASIC PURPOSE

A clinically competent, Licensed Practical Nurse (LPN) delivers care to patients in their place of residence through the nursing process of data collection, planning, implementation and evaluation. The nurse follows through with the plan of care established by the registered professional nurse and effectively interacts with the patients, caregivers, and other members of the health care team while maintaining established standards of nursing.

PRIMARY RESPONSIBILITIES

• Perform ongoing patient visits.
• Demonstrate proficiency in the data collection of the patient's physical, psychosocial, emotional, family and support system and environmental status for appropriateness for home care.
• Follow the written plan of care that was developed by the registered, professional nurse in conjunction with other members of the health care team based on medical and nursing diagnoses, identified patient care needs and standards of care.
• Identify patient care needs based on ongoing home visits and report these to the registered nurse.
• In conjunction with the registered nurse, identify expected outcomes and establish realistic goals for patient and caregivers based on data collection during home visits.
• In conjunction with the registered nurse, determine nursing interventions and teaching required to achieve each expected outcome and goal.
• Assist in coordinating care and service provided to patient by other members of the health care team including agency employees, other agencies, and vendors.
• Comply with Home Health documentation/communication expectations.
• Comply with Home Health policies and procedures related to Infection Control, including handwashing, body substance precautions, disposal of medically related material, post‑exposure testing and prophylaxis, cleaning, disinfecting and redistributing of medical equipment and supplies.

INTERVENTION

• Perform skilled nursing procedures as ordered by the patient's physician in accordance with Home Health policies and procedures and LPN Practice Act.
• Provide education to patient and caregivers regarding medical condition, medications, and nutritional requirements.
• Demonstrate proficiency in performance of nursing skills necessary to care for patients.
• Document relevant and accurate patient information in the clinical record according to Home Care policies and procedures.
• Submit clinical notes the day of the visit and within 48 hours of the visit.
• Record vital signs on every billable visit or document why they were not taken; include weights and pain as required.
• Measure all wounds on each visit.
• Write verbal orders on the date the order is received and mail to physician.
• Document treatments, care procedures and medication profiles and update when physician orders change.
• Update patient plan of care as needed.
• Listen to voicemail prior to and after shift.
• Communicate all pertinent patient care and scheduling information to supervisor daily.
• Effectively order and use supplies in accordance with payer requirements.
• Use appropriate lab slips and specimen containers and complete labels according to pay source requirements.
• Demonstrate safe use of equipment and good body mechanics.
• Demonstrate ability to organize and prioritize assignments.

EVALUATION OF OUTCOMES OF CARE

• Document patient and/or caregiver response to care, interventions and/or teaching.
• Document patient progress toward outcomes of care.
• Modify plan of care with the registered nurse based on ongoing care and data collection.
• Review and revise patient plan of care at least every 60 days or when significant change occurs.
• Phone patient's physician immediately with significant changes in condition.
• Report significant findings during visits to supervisor, registered nurse and other agency employees.
• Attend required coordination meetings and educational material sessions.
• Present accurate, concise reports at team meetings or when providing patient updates.
• Make referrals to other members of the health care team as appropriate.
• Complete discipline discharges on the day of the discharge visit or when notified; submit documentation within 48 hours.
• Complete agency discharges when no other services are active.
• Sign discharge instruction sheet at visit (preferred) or mail to patient.
• Notify physician of discharge and complete verbal order if not included on admission.
• Complete discharge summaries within 48 hours of patient discharge.

RESOURCE UTILIZATION

• Ascertain knowledge of patient pay source and make visits accordingly.
• Ensure managed care patients have authorizations prior to visits.
• Submit schedules to supervisor in advance or inform supervisor of availability for work according to agency policy.

LEADERSHIP

• Adhere to the Code of Ethical Behavior.
• Comply with Home Health policies and procedures.
• Demonstrate behavior that facilitates the smooth operation of the home health agency.
• Maintain confidentiality of all patient and agency information following HIPAA guidelines.
• Demonstrate awareness of own behavior and its effect on own/agency productivity and effectiveness.
• Provide complete and accurate information for the weekend and holiday visit schedule.
• Assume responsibility for weekend and holiday and evening visits as assigned.
• Participate in on‑call rotation as required.

PROFESSIONALISM

• Demonstrate responsibility in the role of Licensed Practical Nurse.
• Participate in agency activities which promote quality patient care and/or are part of the agency's performance improvement program.
• Maintain current knowledge of nursing and home health through seminars, in‑services, and independent professional reading.

• Adhere to agency dress code/uniform policy and maintain a professional image.

PHYSICAL REQUIREMENTS

This position requires the LPN to work in a variety of environments as the nurse visits patients in their homes. The nurse must travel to and visit patients’ homes in varying weather and road conditions, use a motor vehicle, navigate stairs and other physical barriers, and handle stressful situations.

EDUCATION AND EXPERIENCE

• Must be a graduate of an accredited Licensed Practical Nursing program.
• One year of experience in acute care; previous home health experience preferred.
• Must possess current license as a Licensed Practical Nurse from the State of Ohio or be in process of obtaining one.
• Must possess valid driver's license, automobile in good working condition and insurance.
• Maintain current CPR certification.
