Right at Home in Newton, MA is seeking a full-time Licensed Practical Nurse to join our in-home care team. You will collaborate with office staff, field caregivers and families to ensure the highest quality service for our clients.

Responsibilities include ongoing in-person oversight of care, coordinating plans with families, supervising caregivers, maintaining client records in WellSky, and representing the company professionally in the community.

Qualifications

• Active LPN license in good standing.
• Valid driver’s license and insured vehicle.
• Experience in Home Care is preferred but not required.

Responsibilities

• Oversee client care delivery in person and provide follow-up to family members and caregivers.
• Communicate with senior living professionals and elder care advocates.
• Conduct client and family consultations to identify needs, routines and preferences related to care plans.
• Supervise, mentor and support caregivers.
• Maintain complete, accurate and timely client records in WellSky.
• Represent the company in communities and facilities with positivity and professionalism.
• Perform general office and clerical duties.
