Job summary

Home Helpers Home Care of Alpharetta & North Atlanta Suburbs is seeking an LPN Home Care Coordinator to join our clinical and client care team. This part-time role blends nursing knowledge with care coordination and caregiver training.

You will conduct client visits, support new starts, monitor conditions, and provide hands-on clinical support within LPN scope. Strong communication, teaching, and independence are essential.

Qualifications

• Current Georgia LPN license required.
• Experience in home care, home health or related field preferred.
• Strong clinical judgment and communication skills.

Responsibilities

• Conduct client home visits, wellness visits and follow-up.
• Assist with new client starts and changes in care.
• Monitor client conditions and communicate concerns to the clinical team.
• Assist with new caregiver orientations, training and skills assessments.
• Provide caregiver education, coaching and clinical support.
• Ensure caregivers follow client care plans and maintain quality.
• Review documentation and support regulatory compliance.
• Provide hands-on clinical support within LPN scope when needed.
