Woodlands at Canterfield - Luxury Assisted Living & Memory Care is seeking a dedicated Licensed Practical Nurse (LPN) to provide direct nursing care to residents and to serve as a health care consultant within the community. The role includes medication administration, wound care, and collaboration with an interdisciplinary team to ensure holistic resident well-being.

Qualified candidates will have an accredited nursing degree and an active LPN license in the state.

Qualifications

• Nursing degree from an accredited college or university.
• Current unrestricted LPN license in practicing state.
• 1+ years of nursing experience preferred.
• Ability to demonstrate compassion when working with residents.

Responsibilities

• Provide direct nursing care for residents and general health-related consultation.
• Follow the nursing model to promote holistic care for residents.
• Assess changes in residents’ status and notify physician and family, documenting accordingly.
• Prepare and administer medications as per physician orders and observe for adverse effects.
• Perform wound care and ensure timely documentation.
• Collaborate with interdisciplinary team to meet residents’ needs holistically.
• Report incidents or unusual occurrences to leadership and participate in surveys as required.
• Maintain adequate stock of medications, supplies and notify relevant personnel of needs.
