Job summary

UVM Health - Home Health & Hospice is seeking a skilled LPN for home-based, high-tech nursing care under an established plan of care. Evening, weekend or holiday shifts may be required.

Minimum qualifications include an Associates degree or higher in nursing, 2+ years in home health/hospice/high-tech nursing, and current Vermont LPN licensure or compact licensure. Strong English communication and professional conduct are a must.

Qualifications

• Associates degree or higher in nursing.
• Minimum two years of directly related home/public health, hospice and/or high tech nursing experience.
• Current LPN licensure or Vermont-recognized compact licensure.

Responsibilities

• Provide direct high tech nursing care to patients per established nursing plan of care.
• Communicate with family members and educate them about patient status and care.
• Record and report patient status to clinical staff and case manager as appropriate.
• Adhere to protocols and professional standards in nursing care and documentation.
