Job summary

EON, Inc. is seeking a licensed Practical Nurse (LPN) to join our supportive team in Bemidji, MN. This full-time, non-exempt role involves direct care, on-call responsibilities, and collaboration with multiple services to ensure high-quality support for persons served.

The ideal candidate has an active LPN license, experience in homecare or related services, and strong communication skills in English. Benefits include ESOP, health coverage, 401(k), and other employee resources.

Qualifications

• The candidate must be a currently licensed LPN.
• Experience in homecare, day services, and 245D non-residential services is preferred.
• Strong oral and written communication skills in English.
• Ability to read and follow Department of Human Services and Department of Health rules.

Responsibilities

• To perform direct care as scheduled by the CSS.
• Assist with developing assessments, outcomes, progress and health reports.
• Communicate professionally with the support team.
• Assist in orientation and supervision of new staff.
• Oversee site management and medical appointments as needed.
