Job summary

Discovery Senior Living is seeking a Licensed Practical Nurse (LPN) to provide hands-on nursing care in a senior living community. You will follow individualized care plans, monitor residents, administer medications, and collaborate with an interdisciplinary team to ensure safety, dignity, and well-being.

Responsibilities include assessments, care coordination, documentation, and participation in quality initiatives.

Qualifications

  • Current LPN license (or RN per state regulations)
  • Minimum of two years Licensed Nurse experience
  • Experience in assisted living, long-term care, or skilled nursing preferred
  • Strong clinical judgment, problem-solving and decision-making
  • Effective organizational, time management, and communication skills
  • Ability to manage multiple priorities and delegate appropriately
  • Commitment to seniors and person-centered care
  • Ability to work nights, weekends, and flexible schedules as required
  • Proficiency with electronic documentation systems and basic computer applications
  • Ability to respond to medical emergencies

Responsibilities

As a Licensed Practical Nurse (LPN), you’ll play an essential role in enriching the lives of seniors by providing compassionate, high-quality nursing care while supporting resident safety, dignity, and well-being every single day.

Your Role

As the Licensed Practical Nurse (LPN), you provide hands‑on nursing care in accordance with each resident’s individualized care plan. You support comprehensive health and psychosocial needs, monitor changes in condition, coordinate services, and collaborate closely with interdisciplinary team members to promote optimal resident outcomes in a community setting.

What You’ll Do
  • Provide nursing care to residents in accordance with established care plans, policies, and procedures.
  • Conduct ongoing assessments of residents’ physical and psychosocial status and monitor for changes in condition.
  • Administer medications and treatments as prescribed and document appropriately.
  • Coordinate and communicate resident care needs with physicians, families, pharmacies, and other care providers.
  • Support health promotion, disease prevention, and wellness initiatives within the community.
  • Ensure accurate, timely, and compliant clinical documentation.
  • Assist with care planning, service plan updates, and resident evaluations as required.
  • Collaborate with caregivers, wellness team members, and leadership to ensure consistent, high-quality care delivery.
  • Respond to resident concerns, incidents, and emergencies using sound clinical judgment.
  • Support training and guidance for care staff as appropriate.
  • Ensure the community adheres to applicable federal, state, and local regulations related to resident care and services.
  • Participate in quality assurance initiatives, audits, and compliance activities as needed.
  • Perform other duties as assigned.
Qualifications
  • Current Licensed Practical Nurse (LPN) license, or a Professional Registered Nurse as per state regulations
  • Minimum of two (2) years of Licensed Nurse experience required.
  • Experience in assisted living, long-term care, or a skilled nursing environment preferred.
  • Strong clinical judgment, problem-solving, and decision-making skills.
  • Effective organizational, time management, and communication skills.
  • Ability to manage multiple priorities and delegate appropriately.
  • Demonstrated commitment to seniors and person-centered care.
  • Ability to work nights, weekends, and flexible schedules as required by the community.
  • Proficiency with electronic documentation systems and basic computer applications.

  • Exposure to bloodborne pathogens and infectious disease.
  • Ability to lift or assist residents as required by care plans.
