Job summary

RHA Health Services, LLC is seeking a Licensed Practical Nurse (LPN) for a full-time position in a Macon, GA group home setting. The role focuses on holistic nursing care, data collection, and coordination of preventive medical services in collaboration with an Interdisciplinary Team.

The LPN will document care, support physicians during on-site clinics, train unlicensed staff per state rules, and ensure medication safety and regulatory compliance.

Qualifications

• Graduate from an accredited licensed nurse program.
• Current LPN license with no restrictions to practice nursing.
• Current CPR Certification or eligibility to certify for CPR.
• Possess credentials required by Federal or State law.
• Must communicate effectively with Interdisciplinary Team Members and the people supported and their families.
• Must be able to work independently, be organized, and prioritize tasks.
• Must be at least 18 years of age.

Responsibilities

• Provide holistic nursing care to individuals with intellectual and developmental disabilities.
• Observe, assist with gathering health data, and report needs to the RN.
• Coordinate preventive and routine medical care for people supported.
• Document nursing care provided.
• Assist physicians with on-site medical clinics where applicable.
• Provide health-related trainings to unlicensed personnel per state regulations.
• Conduct medication pass observations to ensure staff competency.
• Conduct medication closest assessments.
• Perform QA checks to ensure unlicensed personnel perform health tasks as assigned.
• Observe for abnormal health findings and report to physician.
• Communicate with physicians, pharmacies, medical suppliers, and the Interdisciplinary Team.
• Advocate for people supported and develop community supports to promote health.
