Job summary

The Suffield House Rehabilitation and Healthcare Center is seeking a licensed LPN in Connecticut for full-time, part-time, or per diem roles across shifts, with preference for 1st shift. A shift differential applies for 2nd/3rd shifts and weekend hours, in a modern, supportive facility focused on geriatric care and rehabilitation.

Qualified candidates will have a Connecticut LPN license, CPR certification, English proficiency, and computer skills.

Qualifications

• LPN licensed in Connecticut.
• Minimum of one year education or experience in geriatric nursing or rehabilitation preferred.
• CPR certified.
• Ability to read, write and comprehend English; follow oral and written instructions.
• Fluent knowledge of the MS Office suite and other office equipment.
• Ability to work hours as scheduled based on position requirements.

Responsibilities

• Administers medication and treatments according to physician orders.
• Transcribes, documents, and implements physician orders.
• Collects data to assist in nursing assessments during each shift.
• Gives daily shift report to CNAs assigned to the unit prior to the start of the shift.
• Reviews written daily assignments for each resident on assigned unit.
• Ensures assignments are completed before the end of the shift.
• Gives and takes nursing report on assigned unit; communicates essential information.
• Makes rounds on assigned residents at least every two hours.
• Collaborates with Care Plan Coordinator to develop and maintain nursing care plans.
• Attends interdisciplinary resident care conferences as requested.
• Assists attending physician with rounds and procedures as needed.
• Participates in yearly evaluations of nursing assistants and documents performance.
• Assists Staff Development with orientation and on-the-job training.
• Collaborates with nursing supervisor on disciplinary concerns for nursing assistants.
• Notifies nursing supervisor, resident's physician, and family of changes or incidents.
• Assumes duties assigned by RN Supervisor/ADNS for quality resident care.
