Job summary
Rockville Center is seeking a Licensed Practical Nurse to join our skilled nursing facility team. The LPN will provide direct patient care, administer medications (including IVs as allowed), monitor outcomes, and educate residents and families.
The position requires an active NY LPN license and alternating weekends, with shifts including 7AM-3PM and the potential for weekend requirements. We offer competitive wages and a comprehensive benefits package, including medical, dental and vision
Qualifications
- Current LPN license
- Willingness to work alternating weekends and possible shifts
Responsibilities
- Provide care for residents in SNF and Long Term Care units.
- Administer medications, including IVs as permitted.
- Take and record vital signs accurately.
- Provide resident care and monitor treatment outcomes.
- Educate residents and families on care plans.
- Communicate with physicians, NPs, families and health care team.
- Supervise CNAs and assist with care coordination.
