Rock Haven Campus in Wisconsin is hiring a Licensed Practical Nurse (LPN) for the day shift (6:00 a.m. – 2:30 p.m.), assigned to a neighborhood with 16 to 32 residents.

The LPN will deliver medications, monitor patient care, and support the RN and nursing assistants within a long-term care setting. The position requires current Wisconsin LPN licensure and CPR certification, with priority given to those with long-term care experience.

• Current theory base in medical, geriatric and psychiatric nursing.
• Current theory in medication administration including psychoactive medications.
• Knowledge of needs of client populations and treatment sources.
• Ability to communicate effectively orally and in writing.
• Ability to effectively present information and respond to questions from managers, employees, residents, families, professionals and the general public
• Ability to work well as a team member.
• Skill in reviewing clinical work of others according to professional standards and practice guidelines.
• Demonstrates responsibility and accountability for professional practice and for continued professional growth and self evaluation.
• Ability to maintain professional appearance and conduct at all times.

• Performs related work as assigned.

Mansioni

• Administers medications and performs treatments.
• Performs IV therapy and IV piggyback meds with facility certification.
• Administers PRN medications as indicated and consults with RN.
• Performs dressing changes, tracheostomy care, catheter/ colostomy care and blood sugar checks.
• Conducts EKGs, bladder scans and monitors vital signs.
• Completes lab draws and maintains competency.
• Collects data for nursing assessments and contributes to care plans.
• Assists with resident dining and feeding for high-risk residents.
• May function as team leader during emergencies and code situations.
• Performs additional duties as assigned and completes required training.
