Job summary

Vitruvian Health is seeking a Home Health Care RN Case Manager in Georgia to provide skilled nursing care under the supervision of an RN Case Manager. Duties include chart review, and acting as a back-up for the Intake Coordinator in a home health setting.

The role requires a Georgia LPN license, valid driver's license, and CPR/BLS certification, with experience in geriatric home care preferred.

Qualifications

• Graduate from an accredited vocational school of nursing.
• Must hold current LPN license in Georgia.
• BLS CPR certification required.

Responsibilities

• Assist in implementing the patient's care plan in Home Health settings.
• Provide skilled nursing care under RN supervision and support the Intake Coordinator as needed.
• Cover for Home Health Aides during crisis times to meet regulatory requirements.
• Maintain patient hygiene and a safe, clean home environment.
• Complete chart reviews and essential office tasks as assigned.

JOB SUMMARY

Under the direction and supervision of the Home Health Care RN Case Manager provides skilled nursing care according to the Patient’s Plan of Care. Duties at times may include various office duties such as chart review, and back up for the Intake Coordinator. Other duties may include coverage for Home Health Aides when needed during crisis times and frequencies must be met to meet regulatory compliance. This position is responsible for assisting in the implementation of the patient's care plan and assisting the patient in maintaining good personal hygiene and a safe and clean environment.

JOB QUALIFICATIONS

Education: Graduate from an accredited vocational school of nursing. Licensure: Must have a current driver's license, valid automobile insurance coverage, and current LPN license in the State of Georgia. BLS CPR required Experience: Preferably one-year experience in an institutional setting or provided care to primarily geriatric patients in a home setting.

SKILLS:

CPR Techniques and License. Demonstration of ability to complete personal, environmental and basic skilled and unskilled nursing care to include the following; vital signs, S & A's, Foley catheter insertion, irrigation; administration of fleet and soap suds enema, infection control techniques. Administration of IM & SQ injections, dressing changes, monitoring pain and symptom control. Knowledge of organizational policies, procedures, regulations, and medical equipment utilized in patient care. Knowledge of common safety hazards and precautions for a safe work environment. Skill in modifying documentation to meet intermediary and state documentation requirements. Skill in preparing, maintaining and writing reports. Skill in maintaining effective working relationships with associates, patients, medical staff and the general public. Ability to interpret, adapt, and apply guidelines and procedures. Ability to communicate clearly. Ability to work independently in patients' homes without direct supervision. Ability to drive self on a daily basis.

Varies with patient's home environment. Moderate road and environmental hazards. Requires moderate mental effort in completing paperwork. Requires moderate amount of sitting while driving. Moderate amount of stooping, bending and lifting. Requires manual and finger dexterity and eye-hand coordination. Requires lifting up to 50 lbs. Requires corrected vision and hearing to normal range. Requires ability to distinguish letters or symbols. Requires use of office equipment such as telephones, copiers and faxes.
