Job summary

Vierra Falls Church, a Vierra Communities property, is seeking a full-time LPN for a 7:00 am to 3:30 pm shift in Falls Church, VA. The role involves direct resident care, supervision of non-licensed staff, and collaboration with families and physicians to tailor care plans.

Qualifications

• High school diploma or GED required or equivalent.
• Active/unrestricted LPN license.
• Active CPR/BLS certification.
• Long term care experience preferred.
• 1-2 years experience in skilled nursing preferred.

Responsibilities

• Provide direct resident care and supervise non-licensed care staff in skilled nursing operations and other areas as appropriate.
• Ensure quality assurance and regulatory compliance while delivering high-quality care and service.
• Oversee coordination of residents' health and wellness needs.
• Collaborate with family members and community physicians to address residents' needs.
• Must be able to speak, read and write in English.

Major Duties and Responsibilities (include but are not limited to):

• Responsible for providing direct resident care and supervision of resident care work of non-licensed team members in the skilled nursing operations and other areas throughout the community as appropriate
• Ensuring quality assurance and regulatory compliance while promoting the highest degree of quality care and service
• Overseeing coordination of residents' health and wellness needs
• Working closely with family members and consulting with community physician(s) to ensure residents' needs are being attended to in a personalized way
• Must be able to speak, read and write in English

Required Qualifications:

• High school diploma or GED
• Active/unrestricted LPN license
• Active CPR/BLS
• Long term care experience is preferred
• 1-2 years experience in skilled nursing preferred
• Must pass a criminal background check
