Responsibilities include health assessment support, medication management, resident observation, care coordination, and specialized care in units such as Alzheimer care.

Qualifications

• Active LPN license in Pennsylvania.
• Ability to document accurately and maintain confidentiality.
• Strong communication and teamwork skills.

Responsibilities

• Assist RN with health status checks and documentation.
• Administer medications and verify orders with accurate records.
• Observe residents and report changes to the team.
• Coordinate care with RNs and participate in nursing committees as needed.
• Maintain residents’ hygiene, safety, and infection-control practices.
• Provide focused care for residents in specialty units such as Alzheimer care.
