Job summary

Complete Care - MidAtlantic is seeking an LPN Licensed Practical Nurse to provide direct care to residents recently discharged from hospital or in long-term care. We offer FT/PT options, with sign-on bonuses for FT ($7,500) and PT ($3,750), plus PRN opportunities.

You will administer medications, perform wound care, monitor health status, assist in care planning, collaborate with physicians and therapists, educate families, and maintain detailed health records.

Qualifications

• Active LPN license in Delaware is required.
• Pass background check, drug screen, and professional references.
• Professional & clinically skilled with leadership qualities.

Responsibilities

• Administer medications: oral, topical, IV, and injections.
• Provide wound care, manage pain, and apply dressings.
• Monitor health status and analyze data for changes.
• Perform clinical procedures and catheter/ostomy care.
• Assist in executing individualized care plans.
• Collaborate with physicians, therapists, social workers, and others.
• Educate families and residents about health status and care expectations.
• Maintain detailed documentation of health status and care.
• Oversee CNA team to ensure quality care and teamwork.
