Waterview Hills Rehabilitation & Healthcare in New York state seeks a Licensed Practical Nurse (LPN) to join a high-quality care team. The role covers daily resident care, medication administration, and proactive collaboration with the care team to maintain safe, effective treatment plans.

Qualifications

• Graduate of an approved school of Nursing.
• Licensed & currently registered in New York as an LPN.
• Committed to extraordinary, compassionate care.
• Team player with cooperative spirit for rehab & optimal resident health.
• Excellent communication, customer service, and organization skills.
• COVID-19 vaccinations encouraged but not mandated.

Responsibilities

• Provide daily nursing care to residents within LPN scope.
• Use nursing process and judgment to set care priorities.
• Administer standard nursing treatments and medications; monitor effects.
• Collaborate with care team to update the Nursing Care Plan and review results.
• Monitor and document vitals and residents’ responses.
• Coordinate delivery of care for resident assignments and activities.
