Avera Maryhouse is seeking a Licensed Practical Nurse to provide skilled nursing care to residents on rotating days/evenings/nights. The role includes assessments, interventions, medication administration, and care coordination with physicians and the nursing team.

Qualifikationen

• Active LPN license in the state of practice
• Ability to work rotating shifts (days/evenings/nights)
• Demonstrated nursing assessment and care planning skills
• Proficient in medication administration and documentation

Aufgaben

• Performs all licensed practical nursing functions and services as directed by the RN.
• Completes resident assessments and plans evidence-based nursing interventions.
• Administers medications safely according to physician orders and facility policy (Pyxis system).
• Participates in care planning and ensures plan of care is implemented.
• Maintains professional documentation and communicates care changes to the physician.
• Directs staff in resident care and supports teamwork to meet patient needs.
• Participates in rounds and communicates changes in resident condition using SBAR.
• Pursues education opportunities and participates in orientation for new staff.
