Job summary

Hampden Post Acute is seeking an LPN to join a compassionate, patient-centered care team in Massachusetts. The role supports residents’ physical and emotional needs under the supervision of the Nurse Manager, with opportunities across full-time, part-time, and PRN schedules.

Qualified candidates hold an active MA LPN license, BLS CPR, and demonstrate strong clinical judgment, communication, and attention to detail within a dynamic facility environment.

Qualifications

• Must have active LPN license in MA.
• Valid BLS CPR certification required.
• Ability to manage multiple patients in a fast-paced setting.
• Strong clinical judgment and communication skills.

Responsibilities

• Provide patient care per written plan and physician orders.
• Follow individualized care plans created by Nurse Manager and team.
• Administer medications per care plan and facility policy.
• Monitor residents’ conditions and report changes promptly.
• Maintain accurate documentation per orders and standards.
• Support a safe, respectful and therapeutic environment.
