Brockton Post Acute Care and Healthcare Center in Brockton, MA is seeking an LPN to join our dedicated team. You will deliver resident-focused care under the Nurse Manager, following care plans and physician orders while maintaining accurate documentation.

Qualifications

• Current, unencumbered MA LPN license.
• BLS Certification (CPR) required.
• Ability to care for and manage multiple patients in a fast-paced healthcare environment.
• Strong clinical judgment, communication skills, and attention to detail.

Responsibilities

• Provide patient care in accordance with standards and care plans.
• Follow and implement individualized care plans.
• Administer medications and prescribed treatments per plan.
• Monitor residents' conditions and report changes promptly to the nurse/physician.
• Maintain accurate documentation per orders and regulatory standards.
• Support a safe, respectful environment for residents and families.
