Job summary

Crest Pointe Rehabilitation and Healthcare Center, a 118-bed Sub-Acute and Long-Term Care facility, is seeking a dedicated Licensed Practical Nurse (LPN) to join our compassionate team. This role supports resident well-being with person-centered care delivered in alignment with our core values of Passion, Respect, and Excellence.

We offer full-time and per diem opportunities with shifts 7 AM–3 PM, 3 PM–11 PM, and 11 PM–7 AM.

Qualifications

• Current, unencumbered LPN license in New Jersey.
• Valid BLS Certification (CPR) required.
• Ability to care for and manage multiple patients in a fast-paced healthcare environment.
• Strong clinical judgment, communication skills, and attention to detail.

Responsibilities

• Provide patient care in accordance with accepted standards, plan of care, and physician’s plan of treatment.
• Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team.
• Perform medication administration and prescribed treatments per plan of care and facility policies.
• Monitor residents’ conditions, report changes promptly to supervising staff, and respond to clinical needs.
• Maintain accurate, timely documentation in accordance with orders and regulations.
• Support a safe, respectful, and therapeutic environment for residents and families.
