Qualifications

• LPN licensed by the State or Multistate License.
• CPR Certification required.
• Two years of nursing experience preferred.

Responsibilities

• Admissions, Discharge and Transfers of residents.
• Medication requisitions with pharmacy and physicians.
• Medication administration.
• Infection control and adherence to center policies.
• Collaborate with interdisciplinary team to achieve outcomes.
• Report abnormal changes in residents’ condition.
• Under supervision as per Nurse Practice Act and center policies.
• Provide nursing care for long-term or rehabilitative residents.
• Collect patient data and report relevant observations.
• Implement and evaluate the patient care plan as required by regulations.
• Supervise other nursing staff in selected situations.
• Maintain documentation accuracy and confidentiality.
