Job summary

Holly Hill Healthcare Center in Towson, Maryland, a 75-bed skilled nursing and rehabilitation facility, is seeking compassionate Licensed Practical Nurses (LPNs) to join our clinical team. You will provide direct nursing care, administer medications, monitor resident conditions, and collaborate with an interdisciplinary team to support quality outcomes.

Qualifications

• Current, active Maryland LPN license in good standing.
• Graduate of an accredited practical nursing program.
• Current CPR certification or ability to obtain upon hire.
• Strong clinical assessment, communication, organization, and documentation skills.
• Ability to work effectively in a fast-paced skilled nursing environment.
• Long-term care, rehabilitation, skilled nursing, or memory care experience preferred.

Responsibilities

• Provide direct nursing care to residents per physician orders and care plans.
• Administer medications, treatments, and nursing procedures safely.
• Observe, assess, and document resident conditions and report changes promptly.
• Maintain accurate and timely clinical documentation and resident records.
• Collaborate with physicians, RNs, CNAs/GNAs, rehab staff, and the inter-disciplinary team.
• Participate in admissions, discharges, transfers, care planning, and assessments.
• Support and guide nursing assistants to promote quality resident care.
• Follow infection control, safety and regulatory standards.
• Promote resident rights, dignity, privacy, comfort, independence and quality of life.
