Job summary

Harmony Senior Services is seeking an LPN to provide high-quality nursing care for residents in a Louisville, KY community. You will coordinate physical, social, and emotional aspects of care under physician orders and resident preferences.

The role requires managing medications, ongoing assessments, and strong teamwork with CNAs/RMAs while maintaining accurate records and participating in trainings to ensure the best possible resident outcomes.

Qualifications

• Current LPN license to practice in the state of employment.
• Experience with senior populations, preferably in assisted living or LTC.
• Team player with supervisory experience.
• Strong verbal and written communication skills.
• Excellent time management and organizational abilities.
• Physically capable of lifting up to 50 lbs and frequent movement.

Responsibilities

• Execute the medication management plan and monitor effectiveness.
• Continuously assess resident needs and update relevant staff.
• Maintain documentation and ensure resident safety and wellness.
• Collaborate with the team to meet resident needs and respond to emergencies.
• Provide reports on resident status at shift changes and in the daily log.
• Motivate and direct CNAs/RMAs and participate in trainings.
