Job summary

Delta-T Group Hartford, Inc. is seeking Licensed Practical Nurses to provide services for group homes from Watertown to Torrington, CT. Positions are billed as independent contractor engagements with a $36+ hourly rate.

Responsibilities include administering and monitoring medical needs, maintaining records, and coordinating with health care providers to ensure consistent treatment while supporting residents and maintaining a safe facility environment.

Qualifications

• CT LPN license is required.
• Current CPR certification (in-person; online class not accepted) required.
• Background check including fingerprinting will be conducted.

Responsibilities

• Administer and monitor residents' medical needs per physician orders.
• Maintain relationships with other health care providers for consistent treatment.
• Maintain medical records and ensure thorough documentation.
• Develop professional relationships with facility residents.
• Ensure facility cleanliness, safety, and health.
