Job summary

Central Florida Health Care, Inc. is seeking a Licensed Practical Nurse (LPN) to provide direct and indirect nursing care at our Crystal Lake Elementary location, working under the Nurse Manager and DON.

Responsibilities

• Be familiar with CFHC’s program philosophies, goals and objectives. Be cognizant of and comply with all CFHC’s policies and procedures, as well as state and federal regulations.
• Be competent in performing the duties of a nurse.
• Understand the requirements of CFHC’s Exposure Control Plan. Personally, utilize universal precautions when practicing in a clinical setting.
• Participate in staff outreach and off-site health care programs as requested by the DON, Chief Medical Officer (CMO), represent CFHC in the community.
• Report as needed to DON any pertinent information or situations that impact on patient care or CFHC liability.
• Attend and participate in mandatory CFHC meetings (Center Specific meetings, Corporate meetings, and other meetings as designated by the DON or CMO).
• Demonstrate ability to work cooperatively with other members of the clinical team. Be supportive of coworkers.
• Maintain a neat and professional appearance at all times.
• Carry out any other activities deemed appropriate by the DON or CMO.
• Screen patients according to CFHC’s protocols and perform other procedures directed by the provider, Nurse Manager, or DON.
• Accurately obtain patient’s medical history.
• Perform accurate charting in patient’s medical record.
• Perform waived lab analysis (e.g., urinalysis, pregnancy test, hemoglobin, glucose, strep, flu)
• Administer injections according to position requirements (subcutaneous, intramuscular, intradermal).
• Prepare and administer vaccines/medications as directed by provider.
• Perform procedures, as ordered (e.g., nebulizer treatments, ear lavage).
• Counsel patients per treatment plan (diet, exercise, medication).
• Understand all emergency protocols and respond accordingly.
• Maintain exam rooms and work areas (stock, clean, disinfect).
• Maintain strict infection control (sterilizing instruments, disposal).
• Accurately manage appointments in the system and follow up on attendance.
• Understand and assist in referrals to outside services.
• Complete and ensure accuracy of clinical logs.
• Maintain EMR notes and communicate patient updates to providers.
• Complete duties appropriate to age groups served.
• Other duties as assigned.

• Accurately make/cancel/reschedule appointments in the computer appointment system. Follow-up on appointment to determine patient compliance and contact patients to reschedule (no-show policy).


• Understand and assist in the referral process of patients to outside services or providers, follow-up on patient referrals.


• Complete and ensure accuracy of clinical logs (ex: hospital, temperature, medication/ vaccine administration).


• Maintain patients concerns within electronic medical records (EHR), communicate with patients via telephone, the patient portal, and other modalities; keep providers and other team members updated regarding clinical changes.


• Complete all duties, responsibilities, and competencies in a manner that is appropriate to the age groups to whom care/services are being provided.


• Other duties as assigned by the clinician, Nurse Manager, DON, or CMO.

PHYSICAL REQUIREMENTS:

• Requires 80% or more time spent standing/walking


• Independently mobile


• Ability to lift weight equivalents to what would be required when occasionally assisting and positioning patients, repositioning equipment and lifting supplies
