You will participate in care planning, administer medications, chart patient progress, and collaborate with physicians. Strong English communication, leadership, and decision-making are essential in this on-site position.

Qualifications

• Must hold current, unencumbered RN or LPN license in Illinois and/or Wisconsin.
• Must have CPR certification or obtain within two weeks of employment.
• Must have knowledge of nursing and medical practices and procedures.
• Demonstrates interest/knowledge in elder care.
• Must read, write, and speak English clearly.
• Must be able to make independent decisions when needed.
• Must interact tactfully with patients, families, staff, and public.
• Must have leadership and supervisory abilities.

Responsibilities

• Provide direct nursing care to residents and supervise nursing assistants on day-to-day tasks.
• Ensure care follows federal, state, local standards and facility policies.
• Plan and implement care plans; conduct rounds with physicians; document patient care.
• Administer medications and treatments as ordered.
• Maintain accurate nursing notes and charts.
• Attend care plan conferences and communicate with families.
• Perform required assessments at admission, transfer, discharge.
• Respond to emergencies and coordinate with other staff.
