Job summary

Responsibilities

• Provide direct patient care, including administering medications, performing vital signs, and assisting with ADLs.
• Develop and implement individualized care plans in collaboration with the interdisciplinary team.
• Monitor and report changes in resident condition to the nursing team and other healthcare professionals.
• Maintain accurate and up-to-date records of patient care and treatment.
• Assist with resident transfers, ambulations, and other mobility needs.
• Perform other duties as assigned by the nursing team and management.
