Avera Majestic Bluffs Assisted Living is seeking a Licensed Practical Nurse (LPN) for day shift to support a multidisciplinary care team. The role includes coordinating care, documenting assessments, and communicating with residents and families to ensure smooth transitions between levels of care.

The ideal candidate holds a current LPN license and demonstrates strong teamwork, attention to detail, and the ability to work flexible hours including every 6th weekend/holiday rotation.

Qualifications

• LPN license active in the state of practice.
• Ability to coordinate and document patient care with attention to detail.
• Strong communication and teamwork skills.

Responsibilities

• Coordinates staffing and ensures assigned responsibilities are completed.
• Documents Medicare assessments and prepares residents for next care level.
• Communicates resident information clearly to the health care team and families.
• Maintains accurate clinical documentation and lab/diagnostic records.
• Schedules annual assessments and coordinates care transitions.
