Job summary

Fairview Health Services is seeking a Licensed Practical Nurse in Bloomington, MN to provide professional nursing care in an Assisted Living/Memory Care setting (Meadow Woods). The role reports to an RN and follows Minnesota home care regulations, with a 72-hour schedule over two weeks, day shifts, and every 4th weekend.

Ideal candidates hold an MN LPN license and BLS certification, with less than 1 year of experience, and an AS LPN preferred.

Qualifications

• Must hold MN Licensed Practical Nurse (LPN) license.
• Basic Life Support (BLS) certification required.

Responsibilities

• Provide professional nursing care per plan and RN guidance.
• Transcribe and implement orders from practitioners.
• Order medications and supplies timely and assist with administration.
• Maintain clinical charts and documentation.
• Report changes in resident status to RN and participate in assessments.
• Support RN in care planning and adherence to home care regulations.
