Job summary

StoneBridge Senior Living is seeking a Licensed Practical Nurse (LPN) to join our care team in Kentucky. Provide basic medical attention, assess residents’ wellbeing, and collaborate with the clinical staff to deliver compassionate geriatric care.

You will administer medications, monitor residents, document treatments, and respond to requests, ensuring safety and dignity. A state LPN license and CPR certification are required; experience in long-term care is preferred.

Qualifications

• Must have an active LPN license to practice in the state.
• CPR certification is required; nursing specialty certification is a plus.
• Experience in geriatric/long-term care settings preferred.

Responsibilities

• Coordinates nursing care for residents and ensures appropriate nurse aide coverage.
• Administers and documents medications and treatments per physician orders.
• Monitors resident status and responds to requests, documenting changes.
• Conducts nursing assessments and notifies physician of status changes.
• Prepares assignments considering safety and skill mix of staff.
• Keeps accurate narcotics count and secures medication rooms.
