Liberty Center for Rehabilitation and Nursing is hiring an LPN for the 7am-3pm day shift. You will support the nursing team by administering medications, performing treatments, and keeping precise records to ensure high-quality patient care.

Responsibilities include developing individualized care plans with the interdisciplinary team, participating in care conferences, and maintaining a safe, clean environment while communicating effectively with patients and families.

Competenze

• Graduate of an approved LPN program with PA license to practice.
• Willingness to learn and grow in a fast-paced environment.
• Strong communication and interpersonal skills.

Mansioni

• Provide high-quality patient care under the supervision of the licensed nurse, including administering medications, performing treatments, and maintaining accurate records.
• Develop and implement individualized patient care plans in collaboration with the interdisciplinary team.
• Participate in care conferences and contribute to patient goals and objectives.
• Maintain a safe and clean environment for patients, visitors, and staff.
• Communicate effectively with patients, families, and caregivers to ensure seamless care coordination.
