Job summary

Heights Care & Rehab is seeking a skilled Licensed Practical Nurse (LPN) to join our day shift team. Earn $36–$43 per hour, 12-hour day shifts, and opportunities for additional hours. Ideal candidates will provide direct nursing care, administer medications, assess residents, and support CNAs to maintain safe, compassionate care.

Active Colorado LPN license required. We value leadership, reliable teamwork, and clear communication.

Qualifications

• Active Colorado LPN License.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and leadership abilities.
• Dependable, organized, and resident-focused.
• Ability to work independently while collaborating with the care team.
• Skilled nursing or long-term care experience preferred but not required.

Responsibilities

• Provide direct nursing care and clinical oversight during the day shift.
• Administer medications and treatments according to physician orders.
• Assess residents and promptly identify changes in condition.
• Communicate effectively with physicians, families, and interdisciplinary team members.
• Supervise and support CNAs to ensure outstanding resident care.
• Coordinate care and communicate important updates throughout the day.
• Maintain accurate and timely documentation.
• Help create a safe, positive, and compassionate environment for residents and staff.
• Support a smooth transition of care between shifts.

Schedule Availability

We are currently hiring for Wednesday, Thursday, Friday, and Saturday Day Shifts, 6:00 AM–6:00 PM.

Applicants may choose a schedule that fits their needs, with opportunities for additional shifts and overtime available.
