Qualifikationen

• Current Missouri LPN license.
• Graduate of an accredited practical nursing program.
• Strong communication and teamwork skills.
• Ability to thrive in a fast-paced healthcare environment.
• Long-Term Care, Skilled Nursing, Rehabilitation, or Post-Acute Care experience preferred but not required.
• New Graduate LPNs are encouraged to apply.

Aufgaben

• Administer medications and treatments as prescribed.
• Monitor resident conditions and report changes to the RN or physician.
• Provide skilled nursing care in accordance with individualized care plans.
• Perform assessments and assist with care planning.
• Maintain accurate and timely clinical documentation.
• Assist with admissions, discharges, and transfers.
• Provide wound care and treatment services as ordered.
• Collaborate with physicians, therapists, nursing staff, and interdisciplinary team members.
• Ensure compliance with state, federal, and facility regulations.
• Deliver compassionate, resident-centered care every day.
